What is a nationwide criminal and child abuse consent?

sooo there's alot going on in my house right now.. I'm only 12 and my mom was saying alot of mean things about my dad. Me and my daddy are really close I tell him everything and were like bestfriend's we never tell eachother's secerets either lol. But anyways I got mad at my mom or saying the stuff then I was in my room with my friend and I heard my mom saying more stuff to my step dad and tommorow when I go to my dad's I'm gonna tell him everything.. And I was in my living room and on the fireplace I saw two peices of paper and it said nation wide criminal and child abuse consent on the top then you had to fill out stuff and I know some stuff about my step dad that they don't know that I know soooo I was wondering what this paper is and what it means. Like did somebody turn in my mom and step dad or is it something that my mom and stepdad wanna fill out about my step dad's kids because of their step dad? Pleeeaaseee help me ='(
